Seriously though, things fail because of the relationship between the limits imposed on an undertaking and the available resources the pursuer can wield within the bounds of those said limits. Time and options. The two types of limits. A ticking clock and/or the remaining bullets.
Most of the time, exhaustion. Like they say, "Even the best of warriors cannot fight when out of breath."
